Personal data will be collected and stored by Body Revive Pilates at the time of booking a class or when completing a Physical Active Readiness Questionnaire (PAR-Q). This is for your health and safety a as well as for the purposes of meeting insurance company criteria.
Health information is also collected in order to tailor sessions to suit your individual needs.
All information gathered by Body Revive Pilates electronically and on paper is treated as confidential and will not be shared with a third party without your prior consent. Otherwise, it is kept securely online, and paper forms are stored in a locked filing cabinet and only viewed by the teacher of the class. This information will be kept by Body Revive Pilates for 7 years after your last session which is the time stipulated by the insurance company.
You may update or correct your account information and email preferences at any time by contacting Body Revive Pilates directly. Or, if you would like to review how it is used, or wish to request that it is returned to you, please contact Body Revive Pilates. You may of course decline to submit personally identifiable information to Body Revive Pilates, in which case, we may not be able to provide certain services to you.
How Body Revive Pilates uses your Contact Information
When registering with Body Revive Pilates you will be asked to sign a disclaimer. In doing so you are agreeing to your email and phone number being used by us to inform you of class changes, service updates and information directly relating to the service you have booked.
Body Revive Pilates Mailing List
Direct marketing emails informing you of new, future or alternative services we provide are sent through a secure permission-based email system called MailChimp (see their Privacy Policy at www.mailchimp.com). When you purchase a service from Body Revive Pilates you will be invited to join the MailChimp email list. Your subscription preferences may be changed by you at any time.
Phone Numbers
Body Revive Pilates may contact you by phone or text to inform you about class times, changes, cancellations or spaces becoming available in class. If you do not wish to be contacted in this way, please inform us in person or by email.
Emergency Contact Information
Emergency Contact information is collected solely for use in an emergency and, should the need arise, these details will be used by Body Revive instructors to contact your next of kin or be passed on to the emergency service.
Images And Photos
Body Revive Pilates reserves the right to photograph or film during our classes. Prior to using these images for our website and marketing purposes on social media (Facebook, Instagram), we will always ask your verbal or written consent first. We do not use the personal details or names of any client in a photograph on our website, printed publications and social media unless express permission is granted.
Personal Data and Third Parties
Body Revive Pilates does not and will not ever share or sell your data to any third party, Body Revive Pilates does not collect data of clients for any third parties.
We will not disclose your personal information without your consent unless disclosure is either necessary to prevent a threat to life or health, authorised or required by law, reasonably necessary to enforce the law or necessary to investigate a suspected unlawful activity.
Notifying you of Changes to this Notice
If there is a change to this privacy notice or how your data is used, Body Revive Pilates will contact you via e-mail or by written or hard copy notice. Body Revive Pilates reserves the right to determine the form and means of providing notifications to you and you may have opted out of certain means of notification as described in this Privacy Notice.
